Delegable Clinical Nursing Tasks & Procedures

Under Oregon Administrative Rules (OAR 851-047), our Registered Nurses evaluate, train, and delegate routine health maintenance tasks to qualified caregivers.

Insulin & Diabetes Delegation

Subcutaneous insulin administration, continuous glucose monitors (CGM), capillary blood glucose testing, sliding-scale protocols, and hypoglycemia recognition.

✦ Medication Administration

MAR Oversight & PRN Protocols

Oral, topical, ophthalmic, otic, and inhaled medications. Rigorous MAR reconciliation, blister-pack oversight, and individualized PRN psychotropic protocols.

✦ Enteral Nutrition

G-Tube & J-Tube Feedings

Gastrostomy and jejunostomy tube feeding delegation, routine water flush schedules, enteral medication delivery, and stoma site health monitoring.

✦ Skin & Tissue Health

Wound Care & Dressing Changes

Non-sterile wound dressing management, pressure injury staging, barrier cream regimens, surgical site monitoring, and skin tear protocols.

✦ Respiratory Support

Inhalation & Oxygen Therapy

Oxygen concentrator and tank management, CPAP/BiPAP adherence checks, nebulizer treatments, pulse oximetry, and stable tracheostomy care.

Catheter & Ostomy Management

Foley and supra-pubic catheter care, drainage bag hygiene, colostomy/ileostomy appliance changes, and delegated bowel management programs.

A structured, predictable process engineered to keep your home compliant and your residents thriving.

STEP 01

Intake & Clinical Triage

Rapid review of physician orders, medical histories, and resident stability within 24 to 48 hours to confirm delegability.

STEP 02

On-Site Assessment

Comprehensive in-person RN clinical assessment evaluating patient baseline, acuity, and specific care setting suitability.

STEP 03

Caregiver Teaching & Validation

Direct hands-on skills instruction, return demonstration, and signed caregiver competency validation under OSBN Division 47.

STEP 04

Protocols & Reassessment

Delivery of customized written task protocols, survey-ready charts, scheduled supervisory visits, and change-in-condition response.

Before you call

Care starts with a clear conversation. Talk with our team about the nursing support your setting needs and the referral or payment path for the client.

What is nurse delegation?

A registered nurse assesses the client, decides which nursing tasks may be safely delegated, teaches the client's caregivers to perform them, validates that competence in person and keeps monitoring as needs change. It is a continuing nursing relationship, not a one-time sign-off.

Do you replace the caregivers we already have?

No. Delegation works through the caregivers already in place — the nurse teaches, validates and oversees them for the client's specific tasks. Caregivers, families and case managers are colleagues in the same care.

Where does the care happen?

In homes and eligible community-based settings across Multnomah, Washington, and Clackamas counties in the Portland metropolitan area.

Who can contact North Star?

Clients, families, caregivers, case managers, agencies and clinicians can call the referral line to discuss whether the requested nursing support is a fit.

What happens when I call?

We will talk through the requested nursing support, setting, care team and next appropriate step. Every referral is reviewed for clinical fit, authorization and payer requirements.

What helps to have ready when you call?

It helps to know the client's care setting, who provides their care today and whether the client has Medicaid or a private-pay arrangement — but you do not need everything answered to start the conversation.

Do you work with Medicaid and private pay?

Yes. We serve clients with Medicaid and private-pay arrangements. The referral team can talk through the pathway for your setting and the client's needs.

When can services begin?

North Star opens for referrals on Saturday 26 September 2026. Every referral is then reviewed for clinical fit, authorization and payer requirements before care begins.
